Adj A/Prof Dr. Shridhar Iyer is a distinguished general surgeon with subspecialty expertise in hepato-pancreato-biliary (HPB) surgery, liver transplantation, organ transplantation, surgical oncology, and general surgery. He is proficient in minimally invasive, robotic, and general HPB surgeries, and currently practices at Mount Elizabeth Hospital, Singapore. His illustrious career spans over 20 years, marked by innovation, leadership, and a commitment to advancing surgical care.

A Passion for Surgery and  Liver Care

The liver, an enigmatic organ with over 500 vital functions and the unique ability to regenerate, has been at the core of Dr. Iyer’s professional journey. As he puts it, “A lifetime is not enough to understand the organ, and I am privileged to be part of the community that advances this science.” Early in his career, Dr. Iyer found his calling in surgery—an immersive field where he thrives under the high pressure and complexities of the operating room. He often experiences a "state of flow," a mental state described by the late renowned psychologist Mihaly Csikszentmihalyi, as a phase of genuine satisfaction.

Education & Specialized Training

Dr. Iyer graduated from Grant Medical College, Bombay University, India, in 1994. He then underwent surgical oncology training at Tata Memorial Hospital, Mumbai, and was admitted as a Fellow of the Royal College of Surgeons of Edinburgh, UK, in 1999. He completed advanced specialty training in general surgery at the National University Hospital (NUH) in 2005 and was admitted as a Fellow of the College of Surgeons, Academy of Medicine, Singapore. In 2006, he received the Human Manpower Development Programme (HMDP) scholarship to pursue a Fellowship in liver surgery and transplantation at Chang Gung Memorial Hospital, Taiwan

Leadership Roles & Contributions

Dr. Iyer has held numerous leadership positions, including Senior Consultant and Group head in the Division of Hepatobiliary and Pancreatic Surgery at NUHS cluster and Co-Director of the National University Centre for Organ Transplantation (NUCOT). As Surgical Director of the National Liver Transplant Programme since February 2017, he has played a pivotal role in enhancing the programme by developing standardized protocols, optimizing allocation policies, and streamlining national processes.

 

With over 18 years of experience in liver transplantation, Dr. Iyer has contributed significantly to the growth of adult and pediatric living donor liver transplant programmes at NUH. His work includes notable techniques such as living donor liver transplantation in porto-mesenteric thrombosis and Budd-Chiari syndrome with IVC replacement, which have been published in prestigious journals like the American Journal of Transplantation, Transplantation, Surgery, Hepatology, The Lancet Gastroenterology etc. He has shown through both experience and published work—that mid-volume centres can achieve outcomes comparable to high-volume centres with a strong quality assurance framework, despite literature emphasizing a volume-outcome relationship. In addition, he demonstrated with published benchmarking that donor surgery could be performed safely with minimal morbidity. He was involved in development of peritonectomy and HIPEC program with the colorectal surgery team. He pioneered Robotic Hepatobiliary and Pancreatic Surgery at the previous academic medical centre.

Dr. Iyer’s research focuses on hepatobiliary diseases, living donor liver transplantation, and quality improvement. He has secured several competitive grants and has published over 100 peer-reviewed articles and 100 international conference presentations. His work encompasses basic science research, surgical techniques, and quality benchmarking.

Research & Scientific Contributions

Innovations in Surgical Education &Training

Dr. Iyer is dedicated to surgical education and training, having served as Programme Director for the general surgery residency programme at NUH from 2010 to 2018. He successfully led the first Accreditation Council for Graduate Medical Education-International (ACGME-I) accreditation for general surgery residency outside the United States. As the pioneer Program Director, he developed and nurtured the ACGME-I residency programme in general surgery from its inception and established fellowship programmes in HPB surgery and liver transplantation for advanced trainees..

 

A firm believer in learning from every patient, he is committed to making surgery safer through comprehensive training curricula and mentorship. Dr. Iyer has organized numerous workshops, including those focusing on basic suturing, bowel anastomosis, surgical stapling, advanced laparoscopic techniques, and research methodology.

Dr. Iyer’s contributions extend beyond clinical work to organizing events and workshops that bridge surgical education and community awareness. Key events include:

 

  • Liver Disease Awareness Event (Oct 2015): Organized under the auspices of the Hepato-Pancreato-Biliary Association Singapore.

  • LiCAM 2016 Postgraduate HPB Exit Exam Course (Oct 2016): Co-Chairman.

  • Singapore Transplant Games (Feb 25, 2017): Co-Chairman, under the auspices of the Society of Transplantation Singapore.

  • 7th International Workshop on Advanced Digestive Surgery (Feb 27-28, 2017): Co-Chairman, HPB Section.

Dr. Iyer’s vision for the future of transplantation and HPB surgery is driven by innovation, quality improvement, and enhanced patient outcomes. Equipped with leadership training from the NUHS Leadership Milestone Programme (LMP), an Executive MBA, and the Harvard Macy Course, he continues to serve patients, students, postgraduates, and the community with passion and commitment.

Dr. Iyer has been instrumental in the steady growth of clinical services within the HPB division. The number of liver and pancreatic resections has significantly increased over the last five years, with clinical outcomes comparable to the best in the world. He actively participated in the randomized controlled trial for single-incision cholecystectomy and continues to promote research and quality improvement initiatives to enhance patient care.

Professional Affiliations & Achievements

 

Dr. Iyer is a founding member and past president of the Hepato-Pancreato-Biliary Association (Singapore) and currently serves as president of the Society of Transplantation (Singapore). In these roles, he actively fosters collaboration among healthcare professionals and promotes advancements in the field of organ transplantation.

 

Throughout his career, Dr. Iyer has been recognized with several prestigious awards, reflecting his dedication to clinical excellence, education, and research.
